Passport Application Instructions
Read these instructions before completing your passport application. Enter information carefully and make sure it matches your official supporting documents.
1. Personal Information
Complete your personal details exactly as they appear on your official documents.
Enter your legal first name exactly as shown on your birth certificate or other official identification.
Enter your legal middle name if applicable. Leave it blank only if you do not have one.
Enter your legal surname or family name exactly as shown on your supporting documents.
Select your correct date of birth as recorded on your official birth record.
Select the Liberian county where you were born.
Enter the specific city, town, or community where you were born.
Confirm your nationality. The form currently defaults to Liberian.
Select the appropriate gender option shown on the application.
Select your current marital status if applicable.
Enter your current occupation or profession.
2. Parents Information
Provide accurate information for both parents.
Enter your father's legal first name.
Enter your father's middle name if applicable.
Enter your father's legal surname or family name.
Enter your father's nationality.
Select Living or Deceased.
Enter your mother's legal first name.
Enter your mother's middle name if applicable.
Enter your mother's legal surname or family name.
Enter your mother's nationality.
Select Living or Deceased.
3. Contact Information
Provide current contact details so passport officers can reach you if necessary.
Enter an active telephone number where you can be contacted.
Enter a valid email address that you regularly access.
Enter your current residential address.
Enter the city or town where you currently live.
Select your current county of residence.
Enter the full name of a person who may be contacted in an emergency.
Enter the emergency contact person's active telephone number.
4. Passport Information
Choose the passport category and processing service appropriate for your application.
Choose Ordinary, Diplomatic, or Service Passport based on the category for which you qualify.
The current prototype displays Standard Service at $50.
The current prototype displays Express Service at $80.
The final passport fee is calculated securely by the server when the payment record is created.
5. Required Documents
Upload clear, readable files. Poor-quality uploads may delay processing.
Upload a clear passport-style photograph in JPEG, PNG, or WebP format.
Upload a readable copy of your birth certificate. PDF, JPEG, PNG, and WebP files are accepted by the current form.
Upload a clear copy of your national identification document in one of the accepted file formats.
Upload any additional supporting document required for your application. This field is optional unless an officer requests supporting evidence.
6. Submission and Payment
Review the entire application before submitting.
Check names, dates, addresses, parent information, passport type, and uploaded documents before submitting.
When the form is complete, select Submit Application.
After submission, proceed to the payment section and complete the required passport payment.
Keep your payment reference and receipt for your records.
7. Appointment and Biometric Processing
Complete the in-person processing requirements after submission and payment.
Choose an available passport service location, date, and time.
Report to the selected passport office at your scheduled time.
Bring the original supporting documents used during the online application.
Complete the required identity and biometric verification process when instructed by the passport officer.
8. Track, Approval, and Collection
Monitor the application until passport processing is complete.
Use your Citizen Dashboard to monitor your application status.
Passport officers will review the application, documents, payment, and biometric requirements.
When your passport is ready, your application status will change to Ready for Pickup.
Report to the designated collection location with valid identification and complete the controlled passport collection process.
